Body Sculpture

This lady has the classic shape for the full body liposculpture.

 

I find that in order to get the most harmonious contours and natural flowing shape it’s best to treat multiple areas. Many women have the so called double bulge ie the hips and then a bit lower the outer thighs.

 

The blue markings show the areas that will be treated. By removing a lot of fat from the hip area the waist line is lengthened and creates a slimmer appearance. When treating the outer thigh it is important to also take fat from the outer buttocks as often it is the actual buttock fat that is projecting into the thigh.

 

In this lady there is a small collection of fat just below the buttock that has become known as the “banana role.” The best outcome occurs when this layer of fat is also reduced.

 

You can see that I have marked the inner thighs and often the inner knees will be done at the same time again with the goal of obtaining natural lines.

 

This lady can choose between a local anesthetic or general anesthetic. It often comes down to a persons individual choice. When treating many areas at the same time I preffer the light general anesthetic.

Large hips

I’m often asked how much fat will you take off, or they say take it all doctor Lanzer! One area that I do try to take as much fat as I can is the hips area. By doing this I’m able to really change the shape of a patient especially in clothes. I have also found that hips are very safe. There is a small risk of fluid collecting in very large stomachs that we call seromas. I am yet to see this in the hip region. Note the crosses that I marked which indicate the area that I wish to take the maximum amount of fat.

Male Breast Reduction

One of the most common areas that I get asked to treat are male chests. Many men are very self conscious of this problem. This procedure through liposuction helps to flatten the chest area.

The first thing I assess is if the breast area, this is made up of just fat or fat and fibrous breast tissue. If its fat then liposuction alone using the latest method can resolve this issue. The procedure is done with 2 small incisions (nicks) and liposuctioned. If there is tissue then an incision usually about 1 cm along the lower edge of the nipple will be required.

Most patients generally have the procedure awake and probably half of the men I see, have the stomach and flanks done at the same time.

 

A firm garment is worn for 2 weeks following surgery and no gym during this time.

 

Mostly people get back to work in a few days.

 

In about 80% of cases there is also fat on the sides of the breast around the side of the chest. It’s important to remove this area to give the most natural result. In this situation the hi def method is helpful in highlighting the pec’s.

Vaser Hi Definition Liposculpture

This man presented with large breasts and a bulging stomach.

 

I have discussed the chest earlier in this section and the goal here was to get as much fat out of the chest as possible. Using the tumescent method I was able to get his chest very flat.

 

However his stomach needed a different type of approach. If one looks closely, one can see the lines across his upper stomach, where this muscular appearance is evident. By using the new Vaser Hi Def Liposculpture technique my goal was to give him an athetic muscular look. In doing so I will be working with his own muscles to help bring them out. This patient though, was a little more involved as he also had excess fat all over and this became a combined Mega Liposuction and Hi Def Liposculpture procedure.

 

In approaching the Hi Definition Liposculpture we try to highlight the “abds” which run vertical on the stomach, “the linear alba” which is a depression vertically above the umbilicus and then possibly the “oblique and lats” on the sides.

 

I was the”first doctor in Australia” to travel overseas and receive hands on training in the Vaser Hi Def Liposculpture method by the surgeons who discovered the method. Since then I have personally modified the method and have been able to get quicker results and with less post operative care.

 

Having performed many hundreds over the last few years, I have noticed an interesting trend that men are bring their friends and often have this procedure together.

Female Breast Reduction

The initial assessment of large breasts is, how much fat is in the breast and how much reduction does the patient want. Some breasts are almost all fat and some just the opposite. Generally as one gets older, I have found more fat and especially if the person has had children. I make the assessment of fat by feeling the breast and then by studying the mammogram breast x ray. Dr Lanzer her performed hundreds of this procedures over the last decade.

The next decision is how much fat is to be removed. There is an enormous variation in what women believe is actually their breast size. This is probably because manufacturers label the bras so differently. Usually we can reduce from a size G to a size C, although some people want smaller and some larger. I may elect to remove between 200 mls and even 2 litres of fat from each side to get this change. Generally expect weeping for one day and bruising.

 

The advantage of the liposuction method is that essentially the entire procedure is via 2 small incisions. There are no long scars.

 

I’m always asked, will the skin droop, the answer is in almost everyone the skin contracts and the breast lifts. Sometimes that is even 10-15 centremeters. By making so many small tunnels in the fat, the mass is reduced and lifts. Of course the main result for patients I see, is that the breast is so much smaller and lighter.

Cosmetic Face Surgery

This lady shows ideal features that a Dermatologist can improve. When assessing a face cosmetic surgery enquiry, one of my first questions is: Do you have fair skin? Fair skin with blotchy pigmentation and the fine lines around the eyes and the mouth respond best. Generally a disadvantage is that olive skin patients can develop pigmentation this can be treated with laser resurfacing. Generally this risk does not occur in fair skin patients.

We could expect that her skin after the surgery will be smoother, cleaner, and firmer. The skin texture should change significantly.

 

The second area that will respond is her upper eye lids. One can see that the skin is touching her upper eyelashes. This creates a “tired appearance” and also there is no flatform on her upper eyelid to apply her eyeshadow or mascara. Patients often complain saying they look tired, when they are not! In the photo you may also be able to see, that her upper eyelids are heavy. This is a layer of fat that seems to accumulate in the eyelid. With the C02 laser we are able to remove this fat layer, so the eyelid looks rejuvenated.

 

Now look at the sagging of the cheeks. This happens from facial movement and ageing. The patient can consider just having the skin lasering and they can see how much contraction occurs or perhaps look at the skin lifting procedures such as the skin face suture lift or a face lift. In this situation we combined the skin suture lift. The advantage is that it’s just a skin procedure and there is no cutting out of the skin. The skin suture does not last for ever and initially there may be a wrinkle or a fold in front of the ear but generally fades with time. The suture is often combined with liposculpture of the neck

 

Finally notice the lips are not defined and the cheeks have a depression. A filler around the lip margin will make it more defined and appear more youthful. A volume type of filler in the cheeks will reproduce the natural convex shape of the cheeks, that we see in young people.
