What Should I Ask My Surgeon?
Check whether any of the problem areas could be changed with diet or exercise.
Ask where you would be likely to store fat if you were to gain weight after the procedure. Find out where the scars will be and whether the skin will sag in the areas where fat has been removed.
Check that your age and health will not interfere with the procedure.
Ask whether you will need to wear a girdle and if so for how long.
What Do I Need To Know?
Just because a doctor has a scalpel, it does not mean he’s a sculptor. It won’t necessarily get rid of stretch marks or cellulite. Nor will it effect future weight gain or your metabolism.
How Long Does It Take?
Depending on the number of areas worked on, a liposuction procedure can take from one to three hours.
How Much Does It Hurt?
There will be some discomfort for most people undergoing liposuction surgery. It is, however, dependent on the individual.
How Long Will It Last?
Once the liposuction is complete, the fat cells in that particular area of the body are gone forever so if you put on weight, it will be in another part of your body.
What Can Go Wrong?
You could end up with uneven, bumpy contours if fat is taken from areas where the skin is more delicate. It is best to speak with your surgeon about risks involved with the liposuction procedure, as everyone is different.
Alternatives And Prevention?
Changes in diet and exercise may help to catch the problem before it gets out of control. If you struggle with losing weight via these methods, but are not keen on liposuction surgery, speak to your surgeon for other options.

Liposuction has become one of the most cosmetic procedures in the world. The most significant breakthrough was probably the introduction of the wet tumescent method by a Dermatologist some 20 years ago. Dr Lanzer helped convene a conference to teach that method in 1992 and it can be used to remove fat with either the patient awake or asleep. The fluid that is placed in the fat helps soften the fat and essentially stops bleeding. The body can be sculptured and is ideal for small bulges such as seen on the hips and outer thighs of women and stomachs and flanks of men.
New Areas:
The next advance was the recognition that many different areas are responsive to liposuction. What started for saddle bag removal could be replicated in any fatty area.
Full Body Sculpturing:
As the procedure became more common, dermatologists developed the concept of reshaping many areas in one sitting, body liposculpture. This would often include the back, hips , thighs, buttocks, stomach, knees, arms and ankles. Ideally this is done with the patient under a light general anaesthesia.
For The Overweight:
Many people battle weight reduction and are looking for a kick start. A rapid loss of 5-10 litres in one or two procedures can produce a dramatic change in the body shape. Some patients actually enjoy looking at the fat removed! Most multiple procedures in stages are spaced between 1-4 weeks usually.
For The Heavy Breasts:

There are many new cosmetic procedures and treatments which have varying length of how long the result lasts for. Cosmetic Dermatologists and Specialist have been behind the development of new lasers and injections and procedures for both rejuvenation and shaping.
Permanent Face Fillers:
Many women have fillers for lip and cheek enhancement. It replaces the loss of volume that occurs with aging. Doctors stress the importance of keeping fit and eating well to enhance the long term results as fat can come back into other areas. Liposculpture is a form of key hole surgery where the fat is prepared with a watery solution and then suctioned away.
Breast Reduction In Women:
The breast react like other areas of body fat and once the fat is removed the breast becomes lighter and tighter for many years if not forever. Hormonal effects on the breast tissue itself may change the breast size, which would be relevant especially if a teenager was having the reduction.
Suture Lift:

Cellulite is a skin condition caused by enlarged fat cells that become trapped in an inflexible network of fibrous septae located in the hypodermis. Women more often suffer from cellulite because their subcutaneous fat layer is separated by thin vertical columns in contrast to the crisscross network that is found in men. In women with cellulite, these columns contain enlarged cells, and their design allows herniation to be more visible on the skin’s surface .
Cellulite appears when fat cells just under the skin swell or enlarge and the once flexible collagen bands become rigid and firm and no longer stretch properly. This cell structure change is what causes the dimpled
skin known as cellulite.
Contrary to popular belief, the formation of cellulite does not result from poor diet, lack of exercise or increased body fat; cellulite can be apparent on very lean and toned females as well. Instead, several genetic and physical factors contribute to the formation of cellulite.
These include:
Hormonal Factors – Hormones, not exercise or diet, primarily regulate fat storage and metabolism in the subcutaneous fat layer.
Vascular Changes – Increased deposition of GAGs (glycosaminoglycans) exerts pressure on the capillary walls, which leads to vessel permeability and excess fluid in the dermis, adipocytes and lobes of the fibrous septae. The ensuing edema results in vascular compression, decreased venous return and less oxygenation of the tissue in the fibrous septae.
Connective Tissue Changes – When the fibrous septae housing the enlarged fat cells becomes stiff and cannot stretch, herniation of subcutaneous fat cells extrudes into the dermis, causing bumps and dimpling.

One of the most confusing aspects of cosmetic surgery is terminology. Liposuction, Liposculpture, Mega liposuction, Laser liposuction, Vaser liposuction, lipolysis and such terms are often used and can hard to distinguish. The truth is that in many situations they mean the same thing. Liposuction is the removal of fat via a suction apparatus. The liposculpture is reshaping. A surgeon will always be sculpturing whilst he sucks and therefore liposculpture is really the same practically as liposuction. It just sound better! Dr Lanzer uses the word megaliposuction when he uses liposuction to assist in patients who are overweight. By removing many litres and changing the patients shape they stimulated to change their lifestyle also. With meg liposuction many litres of fat can be removed from many areas. Some patients who have mega liposuction have had lap banding or other weight loss procedures.
Dr Lanzer published many years ago a study on safety of large volume liposuction.

This month Dr Lanzer will be conducting a seminar one evening in Sydney on the new method of hi def Liposculpture. Dr Lanzer was the first in Australia to learn this new liposculpture procedure when he went to Japan this year. At the seminar prospective patients will have the opportunity to meet a patient who has had the liposuction and see first hand what can be accomplished. Dr Lanzer uses the new devises of Vaser to help mould and enhance the athletic appearance in both men and women.

Having communicated recently with my colleagues in America it appears the reason that the Vaser Hi Def Liposuction assisted fat transplant of ones own fat is doing so well relates to the type of effect the Vaser Hi Def Liposuction produces on the cell.
The Vaser breaks the connection between the fat cells and therefore allows easier removal with the liposuction or liposculpture. What is interesting is that the cells stay in tact and therefore are healthy to be injected into other places.
A common place to inject fat is in male upper chests to induce the masculine look. Also fat is inected into the face , the back of hands and the lips of the vagina.
Renewed interest has risen in the procedure of fat injection into the breast. Patients can expect to increase one size and when combined with Vaser Hi Def Liposuction around the breast a more enhanced breast can be produced.

Its now 9 months since I gained by certification for Hi Def Liposuction. Having performed some 10,000 liposuctions over the last 17 years I am personally amazed just how much it has changed the way we can liposculpture the fat from the body.
Many patients prefer an athletic look which gives shape and definition to the stomach muscles. The result can be just to have a slight definition of the abds on both sides of the stomach or it can produce a full ” 6 pack” appearance. The first thing I do is have the patient tighten his stomach muscles which shows me the natural muscle definition. I then mark these areas and by selectively removing fat over the tendons between the muscle we can produce the athletic or muscular appearance.
Women also love the groove than we can produce along the two sides of the lower stomach. The results of the Hi Def liposuction can be enhance with the new Vaser device that softens the fat.
The results are often visible in 1-2 weeks yet the full defined result can take a few months. Photographs are now available of our latest results.

Liposuction in obese patients
Liposuction is one of the more common elective surgical procedures in the US and is supposed to be on the increase. There are no reported studies specifically addressing the metabolic sequelae of liposuction in obesity. The aim of the present study was to investigate the role of large-volume liposuction on insulin resistance and circulating inflammatory markers in obese people. Thirty healthy premenopausal obese (body mass index (BMI) from 30 to 45) and 30 age-matched normal weight (BMI<25) women were studied. In obese women, insulin sensitivity, as measured by the Homeostasis Model Assessment (HOMA=fasting plasma glucose x fasting serum insulin divided by 25), as well as serum adiponectin, the novel adipocytokine with insulin sensitising properties, were significantly lower, as compared with nonobese women (p<0.01), indicating insulin resistance; on the contrary, serum concentrations of the proinflammatory cytokines IL-6, IL-18 and TNF-alpha, as well as the sensitive marker of inflammation C-reactive protein, were significantly higher (p<0.01). All obese women were submitted to a single large volume liposuction (superwet technique): the mean aspirate volume was 3540 ml (range 2550-4670), corresponding to a net lipid loss of 2.7+/-0.7 kg (mean+/-SD). After six months of stable body weight after liposuction, women were less insulin resistant (p<0.05), had reduced concentrations of IL-6, IL-18, TNF-alpha and CRP (p<0.05-0.02), and increased serum levels of adiponectin (p<0.02) and HDL-cholesterol (p<0.05). There was a significant correlation between the amount of fat aspirate and changes in HOMA (r=0.28, p<0.05), TNF-alpha (r=0.31, p<0.02), and adiponectin (r=-0.34, p<0.02), as well as between the decrease in TNF-alpha and the increase in adiponectin after the surgical procedure (r=-0.45, p<0.01). Our study demonstrates that liposuction is safe and free of metabolic sequelae in obese women, pending a careful screening of the patient. Moreover, it is associated with amelioration of insulin resistance and reduced circulating markers of vascular inflammation which may help obese subjects to reduce their cardiovascular risk.
